Aortic root rupture during trans-catheter aortic valve implantation.

چکیده
An 86-year-old woman underwent transfemoral aortic valve implantation (23-mm SAPIEN). She required immediate cardiopulmonary resuscitation. Contrast angiography revealed periaortic blood extravasation (Fig. 1, panel C). Immediate operation revealed an aortic tear in the suprannular position (Fig. 1, panel D) that was repaired with direct suture. A 19-mmMitroflowwas implanted. The patient was discharged home on postoperative day 9. www.elsevier.com/locate/ejcts European Journal of Cardio-thoracic Surgery 39 (2011) 788

Trans-catheter Bioprosthetic Heart Valve Implantation in Iran (Tehran Heart Centre Experience)
Introduction: With the development of interventional cardiology in the world, in addition to coronary and aortic diseases, the treatment of heart valve diseases through catheters has recently begun. The treatment of aortic stenosis (which was only possible with open surgery and valve replacement) was first performed in the world in 2002 by Alain Cribier in France with catheter insertion of the ...
متن کاملUnexpected left ventricular free-wall rupture following an aortic catheter-valve implantation.
Our experience with the Sapien trans-apical aortic valve (Edwards Lifesciences Inc., Irvine, CA, USA) has been straightforward without per-procedural mortality except in 1/16 consecutive cases who developed non-apical haemorrhage early after valve implantation. We describe the case of an 84-year-old female carrying a very high operative risk (logistic EuroScore of 44%), who underwent a trans-ap...
متن کاملConservative management of aortic root rupture complicated with cardiac tamponade following transcatheter aortic valve implantation
Aortic root rupture and cardiac tamponade during transcatheter aortic valve implantation is a frightening complication with high mortality rate. A conservative management of this complication could represent an initial strategy, especially in high-risk patients, to avoid emergent cardiac surgery. This conservative management includes: Immediate detection of pericardial effusion by echocardiogra...
متن کاملMonitored anesthesia care with dexmedetomidine in transfemoral percutaneous trans-catheter aortic valve implantation: two cases report
Percutaneous trans-catheter aortic valve implantation (TAVI) is recommended for inoperable patients with severe aortic stenosis at high risk for conventional aortic valve replacement. Originally, TAVI was mostly performed under general anesthesia. Here we describe two cases of transfemoral TAVI performed under monitored anesthesia care (MAC) with dexmedetomidine. Dexmedetomidine provides sedati...
متن کاملFirst successful trans-catheter aortic valve implantation through ascending aorta using Edwards SAPIEN THV system.
Trans-catheter aortic valve implantation is rapidly becoming an established modality of treatment for patients with critical aortic stenosis who are high risk for conventional aortic valve replacement. This approach also has its limitations; however, with growing experience and improvements in the delivery system, it may be possible to overcome these limitations. We describe a case where conven...
